Incontinence occurs in 1 in 4 women and 1 in 9 men, however only 1 in 12 people pursue treatment to resolve the condition due to embarrassment and lack of education on the treatments available.  Misconceptions associated with incontinence include incontinence occurring naturally with ageing and urine leakage after childbirth.  Most people are unaware that many causes of Incontinence are best treated by a trained Pelvic Health Physiotherapist.

What is incontinence?

Incontinence is defined as the lack of voluntary control over urination and/or fecal matter. Due to hypertonicity (tightness) or hypotonicity (weakness) of the pelvic floor muscles, or a combination of both. These group of muscles join to the pelvic bone and sacrum at the front, back and sides. They support the prostate, rectum, bladder and urethra, and in women they also wrap around the vagina.

A disruption in the normal functioning of these muscles can result in pain and/or dysfunction to the three primary functions of the pelvic floor muscles:

  • Support the internal organs;
  • Provide stability to the back and pelvic girdle; and
  • Prevention of urinary and fecal leakage.


There are various types of incontinence, usually associated with an increase in pressure to the intra-abdominal area. Listed below are the five main types and their symptoms.

  1. Urge incontinence: involuntary sudden loss of bladder control.
  2. Functional incontinence: cognitive or physical impairment causing leakage or urine. Barriers to the use of a toilet, such as environmental factors and psychological unwillingness can also play a role in this type.
  3. Stress incontinence: involuntary loss of bladder control, usually only a small amount, when laughing, sneezing, coughing and when performing exercise or lifting movements.
  4. Overflow incontinence: very full bladder and constant but small urination.
  5. Mixed incontinence: common to have both stress and urge incontinence.


How does this condition develop?

Incontinence usually develops as an outcome of hypertonic (tightness) or hypotonic (weakness) pelvic floor muscles, or a combination of both, resulting in physical and psychological discomfort for the patient.

Incontinence can also occur as a result of:

  • Childbirth
  • Birth defect
  • Medications or surgery
  • An injury
  • As a change to the body occurring with ageing
  • As a result of a disease

Primary Care Advice:

It is best to always first consult with your general practice Physician to determine whether your incontinence is due to issues associated with the pelvic floor muscles or as a result of something else occurring within your body, such as a side effect of medications, or more serious pathology.

Physiotherapy Treatment for Incontinence:

Treatment provided by the Pelvic Health Physiotherapists will involve both internal (if possible) and external examinations to assess which pelvic floor muscles are not functioning as normal. They will then be able to assist in treatments to relax hypertonic (tight) pelvic floor muscles to a normal level and then prescribe exercises to aid in strengthening hypotonic (weak) pelvic floor muscles. Both hypertonic and hypotonic pelvic floor muscles can contribute to the development of incontinence, however with proper treatment, incontinence can be managed and often cured.
